Referral Process
Silverwell Homecare Services, in collaboration with local authorities, health authorities, multi-disciplinary teams, and the wider community, strives to ensure the most suitable placements for service users.

Silverwell Homecare Services is dedicated to ensuring that all referrals for placements are handled thoroughly, promptly, and professionally, in accordance with the Organisation's anti-discriminatory practices. Before offering a placement with the Organisation, a comprehensive assessment will be conducted to ensure that prospective Service Users are informed about the provided services. This allows them to assess if we can meet their needs and aspirations. Information collected for the assessment will be processed and stored in accordance with the Data Protection Act 1998.
Our objective:
Our qualified Referrals Team, with approval from the Registered Manager, will initiate contact to make initial arrangements for assessing the potential service user. They will request a copy of the person's Community Care Assessment and current Care Plan/Reports (where applicable) from the Registered Manager for review.
The obtained information will be forwarded to the identified assessors along with some reference regarding the type of placement required. Any further arrangements for placement visits and changes to assessment dates will be conducted by assessors and communicated directly to the Registered Manager.
If the individual is being assessed for a specific home, the relevant Home Manager is expected to be involved at this stage. The assessment may be carried out jointly in one visit, or the Home Manager may prefer to gather additional information separately to inform the assessment process.
The identified assessors will assess the individual using the standard Inclusive Assessment, Care & Support Plan document of Silverwell Homecare Services, ensuring they gather in-depth information from all relevant individuals involved in the person's care, including family members, current providers, school/college tutors, advocates, and any other healthcare professionals. They will also spend time with the individual to gain their views, wishes, and, most importantly, their needs.
All relevant documentation related to the individual, including current care plans, risk assessments, behavioral guidelines, records of accidents and incidents, and any professional reports, should be requested at this time. This information will be used by the assessors when completing their assessment and will be shared with the Registered Manager.
The completed referral assessment will be forwarded for checking by the Referrals Team (in the first instance) or, in their absence, the Compliance Manager within 48 hours of the assessment date. In the event a decision has been made to offer a placement, a copy will also be forwarded to the Accounts/Finance Department for costing.
As a matter of professional courtesy, the Referrals Team (in the first instance) or, in their absence, the Compliance Manager will liaise with the Registered Manager to inquire and discuss the envisaged support/care required, including one-to-one hours, and advise them of the assessed one-to-one hours to ensure consistency between both parties.
Silverwell Homecare Services does accept emergency referrals, as we recognize that situations sometimes arise which require rapid action.
In these instances, all Silverwell Homecare Services employees should be guided by the spirit of the procedures governing planned admissions. They should gather and supply all necessary information as soon as possible, in a diligent and thorough manner.
